When Diet and Exercise Aren't Enough
You eat well and stay active, yet certain pockets of fat simply refuse to budge. Those persistent areas on the abdomen, flanks, or thighs can undermine how you feel in your clothes and in your own reflection, no matter how disciplined your routine. The frustration is real, and it is rarely about effort.

How CoolSculpting Elite Works
CoolSculpting relies on cryolipolysis, a process that delivers precisely controlled cooling to a localized layer of subcutaneous fat beneath the skin. The CoolSculpting Elite system uses redesigned C-curved applicators for broader coverage and can reduce up to 20 to 25 percent of fat cells in a treated area, with results emerging in as little as four weeks and continuing to develop over about two months.
Why Cold Works
Cells can be injured by both heat and cold. What makes cryolipolysis useful is that fat cells are more susceptible to controlled cooling than many surrounding tissues. CoolSculpting removes heat from a localized fat layer until targeted fat cells sustain a controlled cold injury, while the overlying skin and surrounding structures are largely preserved.
Those cells do not disappear immediately. The cold injury starts a gradual biologic response: the body clears the damaged fat cells over the following weeks and months. That is why contour change develops progressively rather than immediately after treatment.
At first, you may feel intense cold, pulling, pressure, or tingling. As cooling continues, the area typically becomes numb and those sensations usually diminish.
The science is not simply “freezing fat.” It is using controlled cooling that fat cells tolerate less well than the surrounding tissues.
When the Goal Is Contour, Not Weight Loss
CoolSculpting treats localized, pinchable subcutaneous fat, not generalized weight loss. It may fit several different patient situations, depending on the area and the goal.
A Stubborn Localized Bulge
Some patients are at a stable or satisfactory overall weight but have an isolated pocket of pinchable fat that does not match the rest of their body. The goal is localized contour refinement, not a lower number on the scale.
After Meaningful Weight Loss
After substantial weight loss through lifestyle changes, medical weight management, or a GLP-based program, a few localized fat pockets may remain. Further generalized weight loss may not be desirable. CoolSculpting may be considered once overall weight and body composition are reasonably stable.
When the Goal Is Simply Better Contour
Not every patient wants to become extremely lean or athletic. The goal may be a more comfortable waistband, clothing that falls more smoothly, a less noticeable bulge, or a more proportional contour. A particular clothing-size change is not promised.
When Surgery Is More Than the Patient Wants
Appropriately selected patients may want localized fat reduction without incisions, anesthesia, or surgical recovery. CoolSculpting offers a nonsurgical option when the concern and expectations fit the treatment.
Further Refinement
Some patients may choose additional treatment to further refine an area that responded to an initial CoolSculpting treatment. Repeat treatment is not necessary for everyone.
Patient selection matters. CoolSculpting is not designed to directly treat generalized obesity or weight loss, predominantly visceral abdominal fat, significant skin laxity, or structural contour problems unrelated to subcutaneous fat. It is not a substitute for surgery when surgery would be more appropriate.
Dr. Gerrish on Great Day DC: Explains CoolSculpting
Watch Dr. Scott Gerrish explain how CoolSculpting works, who may benefit, and why treatment planning matters when addressing different areas of the body.
What to Expect: Little Downtime, Gradual Change
Treatment Day
Your treatment plan determines the areas and applicators used. You may initially feel intense cold, pulling, pressure, or tingling; these sensations usually lessen as the area becomes numb.
Little to No Downtime
CoolSculpting is nonsurgical, and most patients can return to regular activities afterward. Temporary numbness, tenderness, swelling, bruising, sensitivity, redness, or tingling can occur.
Results Develop Gradually
CoolSculpting does not create an immediate final result. Some patients may begin noticing changes within several weeks, but visible contour improvement develops gradually and can take up to approximately three months to become apparent as the body processes the treated fat cells. Timing and results vary.
Additional Treatment May Provide Further Refinement
Some patients may benefit from additional treatment depending on the area, amount of localized fat, response to the first treatment, and desired degree of contour refinement. There is no fixed number of sessions for every patient.
What a Good CoolSculpting Result Looks Like
Smoother Contour
A localized bulge may become less prominent and blend more naturally with the surrounding body contour.
Better Proportion
The treated area may look more consistent with the rest of your body rather than standing out as an isolated pocket.
Not Necessarily a Different Number on the Scale
CoolSculpting is intended for localized fat reduction and contour refinement. A successful result does not require meaningful overall weight loss.
Sometimes the change is subtle in a photograph but more noticeable in the way clothing sits or how proportionate an area feels relative to the rest of the body. Results vary; visible improvement and clothing-size changes are not guaranteed.
When Fat Reduction Is Only Part of the Plan
CoolSculpting addresses localized subcutaneous fat, but body contour is influenced by more than fat alone. Muscle tone, skin laxity, skin quality, and cellulite can each affect the final appearance of an area. When those concerns are present, another treatment may be planned alongside CoolSculpting or used later as a complementary treatment.
CoolSculpting does not directly tighten loose skin. After meaningful weight loss, including medically managed weight loss, or after localized fat reduction, remaining skin laxity may become more noticeable as there is less underlying volume. This does not necessarily mean CoolSculpting created the laxity; existing skin quality may simply be easier to appreciate.
CoolTone: Muscle Tone and Definition
CoolTone addresses muscle rather than fat. In appropriately selected patients, it may complement CoolSculpting when the goal is not only reducing a localized fat pocket but also improving underlying muscle tone and definition.
Matrix Pro: Skin Tightening and Remodeling
When skin laxity or skin quality becomes part of the concern, Matrix Pro may be considered to address the skin and underlying collagen remodeling rather than the fat layer itself.
UltraClear: Skin Quality and Tightening
UltraClear may also be considered when improving skin quality, collagen remodeling, or tightening is part of the treatment goal. CoolSculpting and UltraClear address different tissue layers, so they may play complementary roles in an overall contouring plan.
The sequence depends on the treatment area, degree of laxity, skin characteristics, amount of fat reduction desired, and individual goals.
The goal is not to combine treatments simply for the sake of doing more. It is to identify which tissue is creating the concern and use the appropriate treatment for each layer. Not every CoolSculpting patient needs combination treatment.
Dr. Gerrish's CoolSculpting Experience
In 2012, Dr. Gerrish developed Treatment to Transformation (T2T), an approach to CoolSculpting treatment planning that moved beyond simply treating an isolated bulge to consider the patient's broader contour and treatment goals. Dr. Gerrish later served for eight years as Medical Director of CoolSculpting University East, helping educate physicians and practices in treatment planning and technique. For you, that means a plan shaped around your overall proportion and balance, with an honest view of when CoolSculpting fits and when another approach may serve you better.

Where This Fits in Your Treatment Strategy
CoolSculpting is selected when localized fat reduction is the priority, as one part of a body-contouring plan rather than a weight-loss treatment. The applicator does not create the result. The treatment plan does. Whether it fits your goals is determined during consultation.
